Response Physiotherapy Guide To Soft Tissue Injury

Day of the Injury - protect

  • Rest - avoid undue stress to the injured area.
  • Ice - 10 minutes every two hours. Ice should be wrapped in a
    damp cloth and never applied directly to the skin.
  • Compress with a firm bandage (not at night).
  • Elevate the injured part above the heart regularly throughout
    the day- at night place a pillow under the mattress to elevate
    the injured part.

One day after injury - encourage good healing

  • Rest combined with stretches to the injured area. The stretches
    should be for 20 seconds at a time and only to the first ?pull?.
  • Ice 10 minutes three times daily
  • Continue to compress
  • Continue to elevate

Two days after injury - improve healing

  • Decrease rest, increase activity
  • Ice five minutes, heat five minutes - two times daily
  • Keep stretching the injury to the ?pull?

Four days after injury - strengthen the injured tissue and improve function

  • Start using the muscle/ligament
  • Leg - balancing on one leg
  • Arm - leaning on the wall with an outstretched arm
  • Functional activities i.e. reaching and lifting

Non acute soft tissue injury

Two weeks after injury and not seen by GP in the very early stage of injury.

Key Points

  1. At this stage, RICE (Rest, Ice, Compression, Elevation) is not as effective.
    Short periods of rest required through the day, but the injured area needs to be reminded of its function. Start using it gently.
  2. 5 mins ice followed by 5 mins heat as chronic swelling/injury often reacts well to the contrast - eases pressure and increases movement.
  3. Compression will help as it can heat swelling that has settled, and can also squeeze some of this swelling out. The easiest compression is a tubigrip. The most effective is a well applied bandage ? firm but allowing the blood to flow (blue/numb limbs means it is to tight)

Exercises


Leg Injury

Balance on one leg; Gentle lunge; Gentle squat; Gentle side to side squat
If these feel comfortable, do some hopping movements.

Arm Injury

  • Reach for an object at different heights (choose a light object to start
    with). Make sure the hand moves first as often the shoulder is first to
    move and this is not useful.
  • If this is comfortable, begin the same exercise with gentle weights and
    increase as necessary.

A pull is a good sensation, but a sharp pain means the injury is not ready to do that much.

Specific exercises will depend on the injured area.
